Communication

Communication

There are a number of ways Royal Perth Hospital sends communication to GPs when one of their patients has been seen. Please ensure your practice details are kept up to date.

Updating GP or Practice Details for Hospital Communication

To check or update your practice details or enrol for electronic or faxed discharge summaries and outpatient letters, please email RPH.GPN.Coordinator@health.wa.gov.au, telephone 9224 3673 or fax 9224 2348 and provide the following information:

  • your Practice details (including Practice name address, phone, fax)
  • a list of all current GPs at your practice including their provider numbers
  • your practice’s EDI code for secure electronic transmission, which can be obtained from your secure messaging provider.

Once updated, your new practice contact details will be available to other public hospitals in WA excluding some public-private hospitals which use different systems.

Please note that GPs and Practices are responsible for ensuring their details are correct and ensuring fax machines or computer systems provide adequate privacy after transmission.

"GP Notify” (the system RPH used to send GPs automated notifications about their patient’s admission, discharge or death by fax or email) is no longer operational. Manual death notifications are being faxed to the patient’s GP and other communications such as discharge summaries or outpatient letters are NOT affected by GP Notify being out of action and continue as usual.

Discharge summaries

Discharge summaries completed for patients who have a My Health Record will be uploaded to their My Health Record unless they request otherwise.

Electronic discharge summaries can be sent from Royal Perth Hospital (RPH) to a patient’s nominated GP using secure electronic messaging or by fax. Hard copies of discharge summaries are only posted to recipients who haven’t been sent a copy by secure messaging or fax. Patients will continue to be given a hard copy if the summary has been finalised before they are discharged.

To enrol for electronic or faxed discharge summaries or to update your practice details, please email RPH.GPN.Coordinator@health.wa.gov.au, telephone 9224 3673 or fax 9224 2348 and provide the following information:

  • your Practice details (including Practice name address, phone, fax)
  • a list of all current GPs at your practice including their provider numbers
  • your practice’s EDI code for secure electronic transmission, which can be obtained from your secure messaging provider.

Contact information

If you experience a problem with receiving discharge summaries electronically, please contact your secure messaging provider’s help desk in the first instance. For further assistance HealthLink can be contacted on 1800 125 036.

Emergency summaries

Communik8 is a summary of an emergency presentation that is faxed to the patients nominated GP.

The notification provides information about:

  • attendance
  • treatment provided
  • future care plans

Outpatient letters

  • For previous outpatient referral letters or results that are not available on the My Health Record, please contact the Freedom of Information Department at  rph.infocentre@health.wa.gov.au. An email or written request is required. Please include the following in your request.
    full patient name
    date of birth
    medical record number (if known)
    information requirements
patient signature (not required if the requesting practice is listed on RPH systems as the patient's current GP).
